Roles & Responsibilities:

Assists in the coordination of all quality improvement activities, including the reporting and documentation of these activities through the quality structure.  Facilitates the identification of opportunities for improvement and assists in the design and ongoing measurement of performance indicators and outcomes.  Conducts case reviews on all serious adverse events (SAEs) and cases identified with potential quality issues; including chart reviews, staff/physician interviews, written case reports and follow-up on corrective action plans.  Assists with Core Measures reviews and entries as assigned within the required timeframes.  Serves as facilitator for assigned departments (and as needed for all departments) and interdisciplinary teams to assist them in designing, measuring, and assessing performance improvement.  Trains and educates staff on all levels regarding the principles of QI and the quality improvement process.  Analyzes QI related data and presents to QI committees and forums as needed.  Provides guidance on compliance with JCAHO and NYS DOH regulations for quality activities.  Performs other departmental duties as required.  Maintains patient/employee confidentiality in the management of information.  Observes the Health Care System’s compliance policies.  Bachelors of Science or Arts in a health related field required.  Minimum 2 years’ experience in a health related field required.  Registered Nurse, Licensed Practical Nurse, or Physician Assistant preferred.  Must have strong verbal, written, and organizational skills.
