Job Title:  Social Worker – Inpatient Rehabilitation


Roles & Responsibilities:

The social worker in this position will provide psychosocial services to a variety of disabled individuals, particularly Traumatic Brain Injured and other cognitively-impaired patients two days a week (Tuesdays and Wednesdays). There is another three day a week social worker and a full-time social worker on the unit. There is a great need for ongoing supportive counseling for patients and families as well as patient advocacy. The discharge planning component requires coordination with numerous community agencies, managed care and insurance companies. The social worker in this position will be a part of a close knit multidisciplinary team of rehabilitation professionals.

On Fridays, the social worker will be responsible for providing psychosocial services with an emphasis on discharge planning for all patients served through the MACE (Mobile Acute Care for the Elderly) interdisciplinary team. 


New York State LMSW.
Excellent organizational, collaborative, and psychosocial assessment skills.
Knowledge of crisis intervention.
Ability to work with multiple hospitals and community systems.
Inpatient hospital experience preferred.
