Job Title:  Social Worker - Dubin Breast Center



Roles & Responsibilities:

The social worker in this position will be part of the Dubin Breast Center and its Breast Health Resource Program. This is continuity of care position where the social worker will follow patients outpatient and during inpatient admissions. The social worker will be an integral part of the multidisciplinary Breast Cancer team, providing psychosocial screening and comprehensive clinical services to optimize patient outcomes. The social worker will assist and navigate patients with breast cancer from diagnosis, during the complex treatment journey and through survivorship. This position includes providing ongoing supportive services and discharge planning for patients admitted to the hospital for treatment of their breast cancer. The social worker must be knowledgeable about the psychosocial impact of disease on individuals and families and offer supportive counseling and education to patients, loved ones, and staff about the impact of a breast cancer diagnosis. Patients diagnosed with metastatic disease may require ongoing care management and education around chronic illness, end of life care and bereavement counseling. This positon incudes being involved in training, teaching, and program development facilitating psycho-education and support groups (some requiring evening hours).


• New York State LMSW or LCSW (LCSW is preferred). 
• Bilingual (Spanish-English) preferred. 
• Experience providing psychosocial services to patients with cancer preferred
• Experience with patients with serious illness preferred
• Excellent clinical, communication, collaborative, and organizational skills
• Skills in rapid psychosocial assessment and focused, short-term treatment.
• Ability to develop and coordinate programs
