Practice Facilitator

Roles & Responsibilities:

The Practice Facilitator guides primary care practices to develop a culture of improvement, identifying and supporting improvement activities that redefine workflows and improve operational and clinical performance. As part of a health system-wide effort to provide value-based care, Practice Facilitators support and coach practices to create meaningful and sustainable change that will enhance operational efficiency and improve clinical outcomes for patients, providers, and staff. In order to achieve these goals, Practice Facilitators utilize a wide range of improvement tools and methodologies that advance Mount Sinai Health System’s continuous improvement culture

  • Develops understanding of localized practice staff roles and workflows. Assesses levels of engagement in practice improvement and identifies systems and processes that impact practice functions.
  • Orients practice providers and staff to the role of a practice facilitator and goals of primary care   practice redesign.
  • Conducts current state assessment of each practice’s operations, applying process and data analysis tools and methodologies to identify improvement opportunities in clinical and administrative operations that drive defined outcome measures.
  • Creates an improvement infrastructure with defined responsibilities such as: setting goals and using data, measuring and monitoring performance, scoping projects effectively, and setting up accountability plans for action items
  • Provides direct coaching support to practices staff to build process improvement capacity within primary care practices to meet defined goals.
  • Collaborates with practice to develop and test proposed workflow improvements and identify metrics for evaluating workflow and staff role changes.
  • Trains practice staff and leadership to track and analyze performance trends and progress on project implementation and deliverables through reports and dashboards that communicate progress toward achieving clinical and operational objectives. Adjusts approach with support from practice leadership team and staff.
  • Develops proficiency in population health information technology platform(s) and guides practice to incorporate into workflows to achieve desired care delivery outcomes.
  • Identifies and shares best practices to coordinate improvement activities among members of the practice site team and the project support team.
  • Supports workgroups and project meetings with guidance from practice leadership and practice facilitation program management, developing trusting working relationships with all partners.
  • Effectively collaborates with all levels of staff, from leadership to front line personnel. Presents findings and recommendations in formal and informal settings to project stakeholders and leadership under guidance of senior team members.
  • Develops, maintains, and disseminates knowledge of health care delivery trends and evidence-based solutions for improving the delivery of care and services.
  • Performs other duties as required.
  • Bachelor’s degree
  • One year experience in health care office environment with demonstrated responsibility for process improvement, quality improvement and strong coaching skills.
  • Microsoft Office suite – intermediate proficiency
  • Knowledge of improvement concepts and ability to learn models and methods (e.g. rapid cycle improvement, quality measurement & reporting) and clinical practice transformation models and methods (e.g. PCMH, Chronic Care Model)
  • Strong written and verbal communication skills, strong analytical skills and ability to work independently or as part of a team
  • Excellent follow-through skills, self-motivated, and detail-oriented, strong organizational skills to manage multiple projects, issues, and priorities effectively and simultaneously with attention to detail
