Job Title:  Social Worker – Neurology


Roles & Responsibilities:

The social worker in this hybrid new full-time position will provide psychosocial services in the FPA outpatient setting for a single busy neurosurgical practice. The social worker will primarily be responsible for identifying patients requiring social work intervention during the initial outpatient neurosurgery office visit and following these patients through their inpatient surgical post-operative course. The social worker will collaborate very closely with the in-patient neurosurgery team once the patient is hospitalized. Responsibilities for the social worker in this role primarily include conducting specialized clinical assessments, providing individual short-term assistance to patients with adjusting to surgical treatment or barriers to compliance with their medical regimen, specific to the peri-operative period and related to spinal surgery. The social worker will be working closely with a neurosurgery practice, focusing on complex spine patients in a fast paced, demanding environment and will participate in team meetings to review and plan patient services. There is a strong focus on program development and patient satisfaction, and the team is seeking a candidate interested in research.


New York State LMSW required, LCSW preferred.
Bilingual (English/Spanish) preferred.
Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care.
Excellent verbal, written, collaborative, clinical and organizational skills.
Ability to develop psychosocial services.
Experience in medical or healthcare setting.
Experience with elder care services, adult care, preferred.
Ability to work independently.
