Roles & Responsibilities:


The Associate Director of Clinical Integration is responsible for execution of collaborative practices that exemplify MSHS’ commitment to patient centered care, value based care and clinician engagement. This individual works system-wide with health system leaders (eg. Across each hospital campus), inter-professional teams of clinicians in the acute and post-acute settings, market leaders and payer groups to continually improve patient care services and transitions in all settings. The Associate Director aligns work with HSO strategy & objectives; partners with leadership on development of standardized care and works with Directors/Managers in identifying opportunities for improvement of care and processes. MSHS goals aligned with this role include development of clinical pathways, improving bundle compliance, ensuring appropriate use of post-acute services and reducing in total cost of care.





·         With senior leaders implements and evolves the MSHS’ Bundles strategy framework for member/patient management and engagement across the continuum of care, recommending interventional strategies as well as operational efficiencies to produce measurable outcomes

    • Provides direction and support for the on-going development of  Bundled Payment Initiatives for the Mount Sinai Health System
    • Provides high-level support to the System CMO, VP for Care Coordination, hospital CMO/CNOs/VPs and relevant Service Chiefs which includes planning, organizing, directing, and executing activities for the Bundled service lines.
    • Aligns multiple departments and staff (Orthopedics, Cardiac Surgery, Rehabilitation, Peri-Operative Services Value Analysis; Business Development) involved in the Bundled Payment program’s clinical, regulatory, finance, compliance, and strategic matters to foster interdepartmental collaboration across the full continuum of care.
    • Keeps abreast of activities involving the BPCI/CJR programs so that issues can be researched and communicated back and forth with the appropriate parties. Utilizes numerous communication strategies to ensure effective information exchange between the department, the division and the institution.
    • Identifies customer KPI’s and manage program operations to deliver customer defined results.
    • Supports culture change and transformation activities with the clinical team s
    • Advises on clinical operations improvement internally and externally
    • Supports the dissemination of technology and clinical solutions across clinical staff and hospital locations
    • Analyzes population-specific performance and advise on best actions for improvement
    • Advises on and participate in MSHS strategy and product roadmap, providing clinical insight and requirements on market changes and client needs
    • Develops and maintain knowledge of contract(s) with customers, as well as working relationship(s) to foster collaboration in data sharing, care coordination, population health management and outcome improvement.

·         Provides leadership in achieving strategic objectives related to the development of clinical pathways and integrative care models with goal of reducing clinical variation and total cost of care

    • In conjunction with Nursing, Medical and Quality Leadership, provides coordination and leadership in execution of new models of care, programmatic activities, clinical interventions and process improvement at each hospital campus.
    • Helps lead and support efforts to reduce avoidable hospitalizations and ED visits by developing and implementing targeted strategies at multiple points of the care continuum

o   Oversees the activities and programs directed at improving the standardization of patient care and outcomes

·         Other accountabilities include, but are not limited to, leading and managing clinically integrated programs, owning deliverables; leading operational and other process and outcome improvement initiatives across various work streams

·         Facilitates the utilization of performance improvement, quality and research in the change and delivery of clinical care and strategic initiatives

  • Drive change management efforts to increase acceptance of initiatives and technology delivery to ensure success.
  • Direct and mentor team of project coordinator professionals
  • Ensure project management staff is tracking key project milestones and adjusting project plans and/or resources to meet the needs of the stakeholder
  • Selects, hires and trains/orients staff, inclusive of RN Clinical Coordinators, other clinicians and administrative personnel. 

·         Develops, monitors and assesses professional and clinical competencies for RN Coordinators in alignment with program goals.

·         Provides technical assistance, resource services, consultation and education. Participates in and conducts process improvement analysis where applicable.

·         Identifies models of best practice in each area under purview. Responsible for extension of the program for new services or areas.

  • Compiles and analyzes of clinical program activity data and makes recommendations for improvement



·         Master’s degree in Nursing, Hospital or Business Administration or related field.

·         Five years clinical experience including progressive management experience

•     Communication and interpersonal skills to work effectively with a wide variety of individuals.

•     Ability to use basic data entry and retrieval systems.

•     Ability to organize information and teach others.

•     Analytic ability to collect and organize information, identifies problems and trends, and formulates and implements corrective plans or programs.

•     Ability to coordinate, prioritized, and allocates resources.

          Physical stamina, dexterity and mental health necessary to coordinate a broad variety of services and programs for designated patient population on a daily basis.




Current New York State nursing licensure as Registered Professional Nurse for a minimum of one year.
