Job Title:  Social Worker – Transplantation (Liver & Kidney)

  

Roles & Responsibilities:

This specialized float position will be a part of the Recanati/Miller Transplant Institute (RMTI) and will provide psychosocial services to adult liver and kidney transplant patients at offsite practices. This will require travel 2-3 times per week to areas outside Manhattan (ie. Long Island, Westchester, Brooklyn, Queens). Generally, for travel outside of the five boroughs, car service is provided. On days where there are no offsite clinics scheduled, this social worker will have responsibilities within the transplant social work coverage group at Mount Sinai Hospital. 

The social worker in this role will be required to conduct psychosocial assessments to determine readiness for transplant. The caseload will be made up of patients who will have unique challenges based on their varied geographic locations. The social worker will become familiar with local services and community resources to assist patients with their access to care. 

This job is varied in location and will require strong organizational skills and a tremendous amount of flexibility. There is an expectation for ongoing program assessment and development. The social worker will be a part of the interdisciplinary transplant team and will take part in recipient review committee meetings for liver and kidney transplant, inpatient rounds and quality improvement projects.

Requirements: 


New York State LMSW.
Excellent clinical, collaborative, and organizational skills.
Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care.
Experience working with substance abuse preferred.
Inpatient experience preferred.
Experience with patients with chronic illnesses preferred.
Excellent presentation and writing skills.
