Job Title: Social Worker – Liver Cancer/Liver Medicine
Roles & Responsibilities:
The social worker in this position is responsible for following adult Liver Cancer and Liver Medicine (non-transplant) patients both inpatient on 9 Center and outpatient in the Recanati Miller Transplant Institute (RMTI) practice setting. The social worker will meet with patients and families to assess needs and follow them through their in and outpatient course of treatment. They will also work with patients and families regarding end of life issues, complications of grief, and linking them with Oncology/Hospice resources. The social worker will be part of the transplant social work coverage.
The social worker will be part of the medical interdisciplinary team to coordinate psychosocial components of patient care including complex discharge planning. The social worker will have responsibilities that include teamwork, committee meetings and QA project participation.
Knowledge of crisis, family, child and systems theories is utilized in an interdisciplinary setting. There is opportunity and expectation to do program development, e.g., group work, patient mentorship program, development of resource guide for patients and their families. There is also an opportunity to develop expertise in various treatment approaches with both acute and chronically ill patients.
New York State LMSW.
Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care both in and outpatient.
Excellent clinical and collaborative skills.
Organizational and program management skills.
Excellent presentation and writing skills.
Oncology experience preferred.
Inpatient experience preferred.
