Job Title: Social Worker – Medicine/Surgery
Roles & Responsibilities:
This position involves the provision of social work services on a 35-bed unit (7E) to both Cardiac and Transcatheter Aortic Valve Replacement (TAVR) patients in the Heart Hospital. The social worker will provide psychosocial counseling to patients and families and discharge planning services for these patients. The discharge planning component requires fast-paced coordination of home care services and facilitation of placements.
The social worker will be an integral part of the Heart Hospital Center and will have responsibilities that include teamwork, committee meetings and participation beyond the purview of the floor assignment. Knowledge of crisis, family and systems theories is utilized in an interdisciplinary setting. The social worker will also be part of a group of cardiac social workers and participate in program development and implementation. Coverage includes other Cardiac units.
New York State LMSW.
Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast paced environment.
Skills in assessment and coordination of resources within hospital and community networks for effective management of patient care.
Excellent organizational, collaborative and psychosocial assessment skills.
Inpatient hospital experience preferred.
