Job Title:  Social Worker – Social Worker – Kidney Transplant


Roles & Responsibilities:

The social worker in this kidney transplant position will be responsible for following kidney transplant patients which involves assessing patients’ suitability for transplant, following patients while hospitalized as well as out patients both pre and post transplant as needed. This is a very fast-paced job that requires juggling the outpatient practice and inpatient work. (S)he will also be a part of medical interdisciplinary team to coordinate psychosocial components of patient care. The social worker will be part of the Recanati/Miller Transplant Institute and have responsibilities that include teamwork, committee meetings, quality improvement, and bi-weekly recipient review meetings. Knowledge of crisis, family and systems theories is utilized in an interdisciplinary setting. Program development is an expectation of the position.


Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care.
Excellent clinical, collaborative, and organizational skills.
Experience working with chronically ill patients preferred.
Hospital experience preferred.
