Job Title: Program Coordinator – Complex Care Management
Roles & Responsibilities:
Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services takes a leadership role in MSHP Care Management. Social workers and are coordinators within MSHP Care Management provide transitional and long-term care coordination services for a large group of individuals who have diverse insurances. The goal of MSHP is to standardize and consolidate complex care coordination for both short-term and long-term interventions.
The role of the Program Coordinator is to provide leadership and supervision to a group of social workers and care coordinators that are deployed throughout the Mount Sinai Health System. The Program Coordinator participates in various administrative and clinical meetings with other MSHP and practice based leadership. The Program Coordinator is responsible for patient and staff data management that informs complex care coordination.
This Program Coordinator position will be based at one of MSHS’s Manhattan campuses (location TBD). The position will primarily involve the supervision of MSHP Care Management Social Workers and Care Coordinators.
Provide professional and administrative leadership for social workers and care coordinators responsible for providing complex care coordination services to high risk patients with diverse insurances.
Participate in program and policy development and monitoring and continuous quality improvement.
Participate in the development and implementation of ongoing staff educational programs.
Represent MSHP at multiple meetings and develop and maintain effective working relationships with a range of Health System departments and physician and administrative leaders.
• New York State LCSW.
• Ability to provide exceptional clinical and administrative supervision and leadership for social workers in highly regulated, outcomes driven enterprise.
• Leadership experience in innovative programs with complex operational and regulatory requirements preferred.
• Exceptional skills in engaging and assessing patients and families and in coordinating of resources within families and hospital and community networks for effective management of patient care.
• Exceptional organizational, collaborative, communication and presentation skills.
• Strong computer literacy: knowledge of Microsoft Office applications, electronic medical record documentation and reporting and ability to learn additional technology systems.
• Hospital experience and experience with care coordination for a high-need Medicare (Fee for Service), Medicaid (Fee for Service and Managed) and the Medicare/Medicaid population preferred.
• Ability to analyze complex data related to patients and staff and develop ideas for implementing programmatic change.
• Strong interest in being part of an innovative initiative to improve the quality and cost of care.
• Experience supervising MSWs, BA-level staff and/or social work interns providing complex psychosocial services preferred.
