Job Title: Social Worker (part-time) – Cardiac
Roles & Responsibilities:
In this position, Social Worker will “float” in adult medicine-surgery. The primary assignment for the float in this position is Cardiac and Surgery. The social worker will be responsible for providing psychosocial services for patients, especially helping patients and families cope with the impact of hospitalization and of acute and chronic illness. There is a strong emphasis on psychosocial assessments and complex discharge planning.
Ability to be extremely flexible and work effectively on multiple units, with diverse staffs, often on short notice.
Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast-paced environment.
Skills in assessment and coordination of resources within hospital and community networks for effective management of patient care, both inpatient and outpatient.
Excellent organizational, collaborative and psychosocial assessment skills.
Inpatient hospital experience preferred.
