Job Title: Social Worker – Emergency Department
RETU patients will be those who present to ED with complaints and diagnoses which have the potential to be evaluated and dispositioned with a high likelihood of discharge within a 24-hour period or those who are waiting definitive diagnostic testing which is expected to take several hours and will determine the patient disposition.
The social worker in this position will facilitate the discharge of patients from RETU. Responsibilities include psychosocial assessment; a full range of psychosocial interventions for patients/families; transitional care; and, when appropriate, follow-up phone calls; and post-discharge intervention. Services will include counseling; crisis intervention, assistance with concrete services (e.g., pharmacy, home care, transportation, entitlements, referral for community services), placement of patients in facilities and close collaboration with the adult ED social workers, social workers in outpatient settings and Care Coordination programs to whom RETU patients may be referred for follow-up as well. This social worker will also be responsible for conducting assessments on patients in RETU who are listed for admission and are awaiting an in-patient bed.
The social worker may cross-cover in the ED and will also be an integral part of TracED (Transitions of Care in the Emergency Department) that includes clinical interventions, teamwork, committee meetings, participation and coverage beyond the assignment. TracED is a novel interdisciplinary care delivery model that targets high risk adult patients with an integrated set of services and innovations implemented in the ED. The social workers in these positions conduct comprehensive psychosocial assessments and a full range of psychosocial interventions for patients/families, transitional care, follow-up phone calls and post discharge intervention. The social workers are also expected to identify patterns of need, develop and implement programs and use and monitor quality improvement methods and metrics to assess the impact of the effectiveness of TracED and the RETU.
This full-time position requires THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month. The schedule will include weekend days on a rotating schedule. The hours are 8 AM – 8:30 PM. Schedules are subject to change based on RETU patient flow.
New York State LMSW.
Bilingual (English/Spanish) required.
Strong interest in being part of innovative initiatives to improve the quality of care for patients who present in ED/RETU.
Excellent clinical skills, with exceptional skills in rapid psychosocial assessment and focused short-term treatment.
Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.
Strong interdisciplinary/collaborative skills and organizational skills
Exceptional skills in engaging patients and families and expertise in coordinating resources within the hospital and community networks for effective management of patient care.
