Job Title: Social Worker – Heart Transplant
Roles & Responsibilities:
This social worker in this position will join the two other Cardiac Transplant/LVAD (Left Ventricular Assistive Device) social workers and will be an integral member of the team. The social worker will be responsible for following cardiac transplant, LVAD and TAH (Total Artificial Heart) patients which involves assessing patients’ suitability for transplantation/LVAD, TAH, following patients while hospitalized as well as out- patients both pre and post-transplant/LVAD/TAH as needed. This is a complex position that requires juggling the clinic and in-patient work. This social worker will also be a part of medical interdisciplinary team to coordinate psychosocial components of patient care. The social worker will have responsibilities that include leading support groups, teamwork, committee meetings, quality improvement and a weekly 7:00 AM recipient review meetings. Knowledge of crisis, family and systems theories is utilized in an interdisciplinary setting. The worker will be a part of the Cardiac Coverage Group.

Excellent clinical, group work and organizational skills.
Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care and expedious safe discharge planning.
Experience in providing individual and group psychosocial services to patients with acute and chronic illness.
Exceptional collaborative and communication skills with multiple staff and units as well as great flexibility.
Experience with anticipatory grief and end-of- life counseling.
Excellent presentation skills.
Ability to develop programs.
