Job Title:  Care Coordinator – Emergency Department

 
Roles & Responsibilities:

This position is one of five Care Coordinator positions in the Adult Emergency Department [ED]. The Care Coordinator for this position will be based in the Emergency Department (ED) at Mount Sinai Hospital. The Care Coordinator will work closely a team of case managers and social workers within the TracED (Transitions of Care in the Emergency Department) program in an effort to prevent 72 hour revisits and readmission to the ED. The Care Coordinator will assess patient’s functional needs, barriers to getting to appointments, and will schedule primary care and specialty appointments for patients. The Care Coordinator will also assist with the ordering of transportation. 

This full-time position requires THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month. The schedule will include weekend days on a rotating schedule. The hours are 8 AM – 8:30 PM. Schedules are subject to change based on ED patient flow.

 


 

Requirements:

• Bachelor’s degree.
• Experience in healthcare or related field is preferred.
Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
Excellent oral and written communication skills and ability to foster a supportive environment for patients.
Bilingual (English/Spanish).
