Job Title: Social Worker – Adult Emergency Department



Roles & Responsibilities:

This position is one of six social worker positions in the Adult Emergency Medical Department [ED]. In this role, the social worker will facilitate the discharge of patients from ED and for early intervention with patients being hospitalized. Responsibilities include crisis intervention (including cases of domestic violence, sexual assault, human trafficking, and death); psychosocial assessments and a full range of psychosocial interventions for patients/families, transitional care, and, when appropriate, follow-up phone calls and post discharge intervention. Services will also include counseling, concrete assistance (e.g., pharmacy, home care, transportation, entitlements, referral for community services) and close collaboration with the social workers in the outpatient settings to whom ED patients may be referred for follow-up. The social worker will also be expected to cross-cover in the Rapid Evaluation & Treatment Unit. 

The social worker will also be an integral part of TracED (Transitions of Care in the Emergency Department) that includes clinical interventions, teamwork, committee meetings, participation and coverage beyond the assignment. TracED is a novel interdisciplinary care delivery model that targets high risk adult patients with an integrated set of services and innovations implemented in the ED. The social workers in these positions conduct comprehensive psychosocial assessments and a full range of psychosocial interventions for patients/families, transitional care, follow-up phone calls and post discharge intervention. The social workers will be closely collaborating with Case Management throughout the day which includes both formal an informal huddles The social workers are also expected to identify patterns of need, develop and implement programs and use and monitor quality improvement methods and metrics to assess the impact of the effectiveness of TracED.

This full-time position requires THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month. The schedule will include weekend days on a rotating schedule. The hours are 8 AM – 8:30 PM. Schedules are subject to change based on Emergency Department patient flow


• New York State LMSW or LCSW. 
• Strong interest in being part of an innovative initiative to improve the quality of care for at risk adult and geriatric patients who present in ED.
• Excellent clinical skills, with exceptional skills in rapid psychosocial assessment, crisis intervention and focused short-term treatment.
• Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.
• Strong interdisciplinary/collaborative skills and organizational skills.
• Exceptional skills in engaging patients and families and expertise in coordinating resources within the hospital and community networks for effective management of patient care.
