Job Title:  Social Worker – Psychiatry Liaison


Roles & Responsibilities:

This is a new, inpatient Social Worker position in Liaison Psychiatry, which will report to the Director of Social Work in Psychiatry. The position is grant-funded with salary limits through December 2020, with the possibility of continued funding based on outcomes. The social worker will work with Liaison Psychiatrists providing services to inpatients outside of Psychiatry and will collaborate with unit social workers in the provision of services to patients. 

Responsibilities of the role include:

• Arrange post-acute psychiatric care: identify, facilitate and obtain insurance authorizations, as necessary, for outpatient mental health services, higher levels of outpatient psychiatric care and community based supports (e.g., mental health case management, partial hospitalization programs, assertive community treatment teams), and inpatient psychiatry transfers. 
• Provide direct clinical work, under the direction of the Liaison Psychiatrist, with patients with non-pharmacological needs, e.g., providing supportive psychotherapy.
• Collaborate with Liaison Psychiatrists and primary team members to develop behavior management plans for challenging patients. 


• New York State LCSW preferred.
• Inpatient psychiatry or medicine/surgery experience.
• Knowledge of community-based psychiatric care options and experience making post-acute psychiatry referrals. 
• Ability to work as part of a multidisciplinary care team, across multiple inpatient units, in a demanding, fast-paced environment, requiring significant attention to detail.
• Excellent organizational, collaborative and clinical skills.
