Job Title: Social Worker


Roles & Responsibilities:

The social worker in this position will work collaboratively with a case manager with identified medically and/or psychosocially high risk patients throughout adult medicine and surgery units for the primary purpose of reducing the length of stay and improving the outcomes and experience for these patients and their families. Initially, it is anticipated that the targeted population of 13-15 patients, expected to be clustered on several units, will be identified weekly, primarily through long-stay rounds, and will be patients who have been in an ICU for 10 or more days, have triggered sepsis and/or are on the respiratory pathway. The social worker/case manager team will receive hand-offs from the unit staff. This team will be responsible for maintaining records on the patients served, reporting and analyzing outcomes, and recommending programmatic changes. The team will have high visibility with the Chief Medical Officer and her staff.


New York State LMSW or LCSW.
Medicine/Surgery inpatient experience.
Exceptional collaborative and communication skills with multiple staff and units. 
Ability to co-lead a new initiative, with the requisite analytical and presentation skills.
