Job Title:  Social Worker (Bilingual)


Roles & Responsibilities:

Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services takes a leadership role in MSHP Care Management, which is providing in person and telephonic short-term and long-term care management services for a large diverse group of individuals who have a variety of insurances that are part of value based contracts. The goal of MSHP Care Management is to provide standardized care management services to meet patients’ medical and psychosocial needs, increase quality and reduce unnecessary health care costs.

The role of the Social Worker within MSHP is to serve as a member of the interdisciplinary care team and to work closely with this team to provide complex care management services to patients at risk for avoidable hospital and emergency department utilization. The goal of MSHP is to standardize and consolidate complex care coordination for both short-term and long-term interventions and this position will evolve to provide these services to patients covered by diverse insurances. The Social Worker will conduct biopsychosocial assessments with patients and family caregivers, identify barriers to seeking medical/behavioral health care and work with the team and patient/family caregivers to develop a care plan to overcome these barriers and improve health outcomes. (S)he will create linkages for patients with community organizations to facilitate adherence to the care plan in the community. The Social Worker is responsible for educating patients/family caregivers on how to navigate the health system. For common patients, the Social Worker communicates with social workers/care coordinators within MSHP and the Health System to ensure collaborative and seamless work. 

The Social Worker in this role will support MSH High Risk Health Home patients, primarily partnering with the Mount Sinai Visiting Doctors (MSVD) Program. MSVD aims to provide high-quality, patient-centered primary and palliative care to homebound patients in Manhattan, offering emotional support and social services to the families and caregivers of homebound patients. This is a specialty Health Home Social Work role, involving direct patient care management services with frail and elderly homebound populations with an emphasis on end-of-life and palliative care. This position will be based at the MSH campus, 19E 98th Street, Manhattan


• New York State LMSW or LCSW. 
• Bilingual (English/Spanish).
• Exceptional skills in engaging patients/families and in assessment and coordination of resources within families and hospital and community networks for effective management of patient care.
• Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
• Interest in being part of an innovative initiative to improve the quality of care for the frail and elderly population by addressing barriers to behavioral health, primary and specialist care, preventable Emergency Department visits and hospital admissions/readmissions.
• Hospital experience and experience with geriatrics, palliative care and/or end-of-life preferred.
• Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
