Within MSHP, the Clinical Quality and Utilization Management team and the members of the Clinical Pharmacy are responsible for medication management quality, cost, utilization and clinical stewardship activities at practice and program level.


Role Summary

MSHP seeks a Clinical Pharmacy Manager to continue to develop and incorporate pharmacy services into the continuum of care coordination to ensure patients receive optimal pharmaceutical care in accordance with the “Triple Aim;” to improve the health, enhance the patient experience of care, and control the per capita cost of care.  The Clinical Pharmacy Manager serves patients by providing comprehensive medication therapy review; develop a collaborative MTM practice, provide pharmacological information to multidisciplinary health care team; monitoring patient drug therapies and support program management and development across all MSHP pharmacy priorities. Clinical Pharmacy Manager that reports to the Director of Pharmacy, Population Health, MSHP. The Clinical Pharmacy Manager will have direct patient care responsibilities aligned with care delivery goals as dictated jointly by MSHP and practice leadership. The manager will have practice management responsibilities including and not limited to clinic-based staff management. Clinical Pharmacy Manager will develop collaborative drug practice agreements with providers (MDs) at practices. Clinical Pharmacy Manager will lead medication management performance improvement initiatives. Clinical Pharmacy Manager is expected to manage pharmacy students and residents as requested. Pharmacist will assist with design, implementation and evaluation of all related pharmacy initiatives.



Delivery of Clinical Pharmacy services with a focus on chronic disease management

·         Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs

·         Serve as a clinical pharmacy resource for the health-system and its affiliated pharmacy services across affiliate and Clinically Integrated Networks.

·         Supports initiatives including but not limited to:

o   DISRP and DSNP patient panel management

o   Comprehensive Medication Review, MTM

o   Medication care plan development

o   Medication Adherence

o   Polypharmacy management

o   Optimizing dosing schedule, cost-effective, formulary coverage and other challenges

o   Medication counseling and education

o   Intervenes and documents pharmacotherapy recommendations

o   High risk management of medications in elderly



Pharmacy Strategy and Innovation

·         Identify, develop and implementation of new pharmacy strategies and innovation pilot projects across responsible clinics.

·         Provide education, training and work direction to clinic staff and practitioners as required

·         Create and effectively manage project work plans, coordinating across multiple stakeholders

o   Physician Leadership

o   Clinic-based multidisciplinary teams

o   Care Coordination teams

o   IT/EHR

·         Drive interdisciplinary meetings, ensuring quality initiatives are accomplished on time

·         Serve as operational support for quality initiatives, contributing to strategy, process design, implementation, and ongoing evaluation and improvement



·         Serve as a preceptor and manages for the PGY-1 residency program and students from local colleges of pharmacy

·         Provides personnel review and provides performance evaluations

·         Manages pharmacy technicians and provides performance evaluations

·         Provide scholarly activities and presentation at local and national meetings to advocate for and raise awareness of population health pharmacy roles and responsibilities



·         Develop and scale MTM program, manage goals of pharmacy integration across MSHS, CIN employed or voluntary practices

·         Support financial, quality and utilization analytics and performance improvement evaluations

·         Serve as pharmacy representative on behalf of MSHP as required at organizational committees

·         Measure MTM service impact and ROI

·         Designs and manages billing services for an ancillary healthcare provider



Education and Experience

·         Pharm.D, PGY-1 Pharmacy Practice Residency, and PGY-2 Ambulatory Care or Community Care Residency preferred

·         Three years hospital experience with clinical training; ambulatory and hospital-based practice settings preferred

·         Analytics capabilities required; experience with healthcare claims analysis, clinical quality measures highly preferred

·         Interest in and demonstrated passion for healthcare required, experience with healthcare companies (e.g., delivery systems, insurance companies, start-ups) a plus


Additional Skills and Qualities

·         MTM experience, CDTM experience, pop-health experience 

·         Effective communicator – excellent written and verbal communication; able to summarize clinical recommendations, practice guidelines, protocol development in a way that simplifies complex ideas and synthesize research into insights
