The Emergency Department (ED) at Mount Sinai Beth Israel aims to improve screening and linkage to care for patients with Hepatitis C infection (HCV), Human Immunodeficiency Virus (HIV) and Substance Use Disorder (SUD). All three of these issues continue to be major public health challenges in New York City with many persons not yet diagnosed and/or engaged in care. Many patients, who may not otherwise engaged with the health system, frequent the ED for acute care needs.  As such, the ED can help fulfill a critical gap by augmenting HCV, HIV and SUD screening and linkage for otherwise difficulty to reach populations.

 

Roles & Responsibilities:

 

As an integral part of Mount Sinai Medical Center’s health care team, Health Educators play a critical role in the screening of patients for HCV, HIV and SUD in the Emergency Department and facilitating their navigation to definitive outpatient care.  Health Educators will have the following roles and responsibilities, although duties may change to accommodate the evolving needs and activities program:

 
  • HCV & HIV Re-Engagement:  Health Educators re-engage all ED patients who decline an initial HCV & HIV test by the nursing staff in an attempt to convince them to agree to test.  These brief semi-structured encounters focus on providing information on HCV/HIV, reasons for testing and encouragement.  This effort requires a candidate with strong inter-personal communication skills who can develop rapport quickly with patients in a challenging clinical environment. 
  • HCV & HIV Linkage Navigation: The Health Educator will engage with and follow up with all patients who screen positive for HCV/HIV in the ED.  Health Educators provide patients with their test results and schedule them for their first outpatient clinic appointment.  These activities require close coordinating with clinic staff, care managers, administrative staff and other personnel. Follow-up may occur over the phone or in person.  Candidates must be able to work effectively in a multidisciplinary team structure.
  • HCV/HIV Data analysis and Program Evaluation Activities: To evaluate the activities of the HCV/HIV screening program candidates must be able to integrate information from different electronic medical record systems and reports into a single database for analysis.  Program progress is monitored during weekly data with the program medical director.  This effort requires moderate to expert data entry and management skills in Microsoft Excel.
  • SUD SBIRT & Harm Reduction Activities

Health Educators will perform advanced screens for patients identified as being at risk for a SUD by the primary nurse.  For patients identified with SUD educators will perform a brief intervention and navigate patients to appropriate care.  To augment these activities Health Educators will provide naloxone intra-nasal kits to those patients identified with opioid use disorder or at risk for opioid overdose.

 

Health Educator duties may also include additional projects and responsibilities, including additional chart review, linkage to care, data management (possibly in REDCap, Excel, or Access), data reporting, and other duties to be determined.

 

The Health Educator will also participate in weekly meetings with the program medical director and project manager.  The Health Educator reports directly report to program medical director with whom they will have regular one-on-one meetings.

The Health Educator may also be involved in outreach activities to expand the public health screening and link to care model to health partners both within and outside of the Mount Sinai Healthcare System. Outreach activities may include: delivering educational and presentations and attending scientific conferences and meetings.

 

Each Health Educator will assume project management responsibilities for one of the ongoing public health programs.  Depending on the program, these responsibilities will include assisting the program director in the planning and execution of research protocols, overseeing data collection and conducting data analyses, ensuring compliance with IRB requirements; training and supervising research assistants; liaison with funding agency including preparation of reports; coordinating the various stages of development of the projects; building and maintaining relationships between community partners and academic personnel; providing administrative support for meetings, recruitment activities and training activities; administering project funds, and assuring timely completion of project activities.

 

This is an ideal position to gain experience in community health, research, and clinical medicine.

 
Additional details about responsibilities:
  • Follow-up with all HCV Antibody positive patients screened for post-test counseling and basic HCV-related education: natural history, prevention of transmission, evaluation, and treatment
  • Schedule patients for first evaluation with on-site HCV specialist
  • Provide reminders to patients prior to their scheduled appointment and check to see if patients adhered to appointment
  • Promote adherence to all HCV-related appointments and send letter and/or telegram for patients lost to follow-up
  • Review monthly data to identify screening rates and newly screened HCV+ patients to be linked
  • Provide HCV education (evaluation, treatment, adherence, and side effect management)
  • Collect all HCV-related data and input in timely manner within databases
    • Maintain all HCV-related databases (e.g. updating data daily, ensuring data security)
    • Report weekly on database updates to HCV team
  • Provide weekly updates to HCV team meeting for problem solving, quality improvement, future grant applications
  • Attend on-site and off-site trainings as needed.
  • Work closely with community contacts to develop and maintain HCV-related networks (e.g. syringe exchange program, Department of Health and Mental Hygiene, Department of Homeless Services, Correctional Health)
  • Collaborate with HCV team members in dissemination activities (abstracts and papers)
  • Develops and presents health education and promotion programs such as training workshops, conferences, in small groups as well as individual sessions
  • Develops operational plans and policies necessary to achieve health education objectives and services
  • Designs and conducts evaluations and diagnostic studies to assess the quality and performance of health education programs
  • Provides program information to the public conducting media campaigns, and/or maintaining program-related web sites
  • Performs other related duties
 
Requirements:
  • Possess a Bachelor of Science in Public Health, Biology, Psychology, Education or related discipline
  • Have at least 1 year of experience in clinical environment
  • Must possess knowledge of community resources and health care processes, structure, and function
  • Prior experience working with marginalized and highly stigmatized populations strongly preferred
  • Bilingual (English and Spanish) strongly preferred
  • Experience in a supervisory role preferred
  • Database management experience
  • Command of Microsoft Office Suite, experience with REDCap and EMR preferred
  • Must demonstrate ability to work effectively in a team environment
  • Exceptionally organized and able to work independently
  • Strong oral and written communication skills
  • Ability to plan a program based on health needs and requirements 
  • Ability to work with a very diverse population and be sensitive to cultural differences 
  • Comfortable speaking in public 
  • Strong independent judgment
  • Flexibility to take on new duties as needed
  • Certification as a Certified Health Educator (CHES) from the National Commission for Health Education Credentialing, Inc. preferred
