Job Title: Social Worker (Medicine/Surgery)
Roles & Responsibilities:
The social worker will be responsible for providing psychosocial services, including discharge planning, for hospitalized patients on GP 9E, primarily with gastrointestinal diseases (such as Crohn’s , ulcerative colitis, G.I. cancers and liver disease) that require surgical intervention. Knowledge of crisis, family and systems theories is utilized in an interdisciplinary setting. Supportive counseling is provided to patients with new and chronic diagnoses, treatment, and end-of- life care. In addition, complex discharge planning is required. The social worker will collaborate as a member of the interdisciplinary team.
The social worker will also be part of a group of surgical social workers and participate in group work, program development and implementation. The social worker is an integral part of the hospital initiatives and has responsibilities that include teamwork, committee meetings and participation beyond the purview of the floor assignments. Coverage includes other medicine/surgery units.
New York State LMSW.
Inpatient hospital experience preferred.
Excellent clinical, collaborative, and organizational skills.
Experience providing psychosocial services to patients with acute and chronic illnesses
Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast paced environment.
