Job Title: Social Worker

  

Roles & Responsibilities:

The social worker in this full-time position will provide case management/psychosocial services, crisis intervention, psychoeducation and group counseling to patients with HIV Disease and HIV negative patients who are at risk for HIV infection and candidates for Post-exposure Prophylaxis (PREP). A team approach is utilized to provide continuous, coordinated care to patients and their families throughout the course of their illness. Responsibilities also include program development; facilitate linkages to community case management and involvement in outreach activities with community-based organizations, such as the AIDS Service Center. The social worker will also be responsible for facilitating the Consumer Advisory Board (CAB).

Requirements: 


New York State LMSW or LCSW. 
Bilingual (English/Spanish) preferred.
Excellent collaborative and clinical skills.
Experience working effectively with LGBT population.
Knowledge of HIV-related psychosocial issues and services.
