Job Title: Social Worker – Pediatrics



Roles & Responsibilities:

The social worker in this position is a crucial member of the Women’s and Children’s Health interdisciplinary team of providers on two pediatric in-patient floors. The social will provide psychosocial services to patients and their families on P5 and P4 South. Facilitation of discharge planning for children through collaboration with other health care team members is a critical aspect of this position, as is provision of mental health services ancillary to medical management of a child, psychosocial support services, and health education related to post-hospital management of chronic and acute conditions. Interdisciplinary collaboration with attending and resident physicians, primary care nursing and the nurse mangers on these services is also a critical and important part of practice. The social worker works collaboratively also with sub-specialty pediatric social workers whose continuity of care patients may be hospitalized and is the liaison between floor staff and sub-specialty teams. This social worker also works closely with the newly established hospitalists program as well as with the Department of Orthopedics pediatric chief to determine and anticipate discharge needs of electively admitted children and families. Strong communication skills and the ability to collaborate with social work colleagues will be essential to this role.


New York LMSW or LCSW.
Bilingual (English/Spanish) preferred.
Strong collaborative and teamwork skills.
Previous experience working with children and families in a hospital setting.
