Job Title: Social Worker – Medicine/Surgery
Roles & Responsibilities:
This position involves the provision of social work services to adult inpatients on 8E which is primarily a surgical unit and includes orthopedics, urology, plastic surgery and Medicine patients. The social worker will work in close collaboration with another 8E Social Worker. The social worker will provide psychosocial counseling to patients and families and discharge planning services for these surgical patients.
The social worker will have responsibilities that include teamwork, early committee meetings, quality improvement initiatives and participation beyond the purview of the floor assignment. Knowledge of crisis, family and systems theories is utilized in an interdisciplinary setting.
New York State LMSW or LCSW.
Inpatient hospital experience preferred.
Excellent organizational, collaborative and psychosocial assessment skills.
Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast paced environment.
