Job Title: Social Worker
Roles & Responsibilities:
The social worker in this position will be responsible for implementing and coordinating safe mother-baby discharges for a wide variety of patients, including those who are unregistered to Mount Sinai, private insurance patients, unassigned ambulatory practice patients, patients at elevated risk for postpartum depression and patients with psychosocial, medical or safety concerns.
• Work closely with interdisciplinary team and attend daily morning rounds.
• Work closely with Mount Sinai’s child protection program as well as coordinate referrals to an extensive list of community resources, e.g., Administration for Children’s Services, NYC Department of Housing, home care agencies.
• Collaborate with social work colleagues in both the outpatient obstetric and pediatric clinics and neonatal intensive care unit for continuity of care and enhanced service delivery.
New York State LMSW.
Bilingual (English & Spanish) required.
Experience with obstetrics preferred.
Experience with complex medical and mental health issues related to pregnancy preferred.
Ability to work in a high pressure and crisis oriented environment.
Strong organizational, collaborative, program planning skills.
