Job Description

A. Clinical/Technical/Service
  1. Assists clinical staff before and after examinations and  help the families to communicate their needs to the staff
  2. Promotes understanding of health needs, the use of the medical care program as well as the social service and educational programs
  3. Prepares examining room and equipment for safety and comfort
  4. Prepares and maintains charts of new and old patients
  5. Assists doctors and nurses during pediatric and/or adult evaluations as well as specialty clinics and emergency visits
  6. Serves as an escort to patients to various places such as the laboratory for blood work, and X-ray Department
  7. Requests patient records from hospital and different agencies involved with patient
  8. Files and Xeroxes of records
  9. Follows-up care of patients regarding recommendations by telephone
  10. Interprets for social service worker in individual casework contacts
  11. Helps families with social problems
  12. Helps families with any problems which interfere with their keeping of appointments (i.e., transportation or child care)
  13. Works with other professional disciplines on the team to secure significant data from them, to share data, to interpret the family reactions and needs and to develop joint planning for the individual patient and family
  14. Reports back to other team members on patient needs and general status of patients known to worker
  15. Serves as an advocate for people unable to negotiate systems within and outside the hospital
  16. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area

    1. Identifies physical, behavioral and emotional characteristics typical for the age group
    2. Modifies approaches based on patient age-specific needs and responses to treatment
    3. Provides care for patients based on age-specific needs
    4. Uses communication techniques which are age appropriate

  17. Participates in the department’s performance improvement activities
  18. Maintains patient/employee confidentiality in the management of information
    C. Educational/Professional Development
  19. Participates in the development of other staff members
  20. Meets regulatory, licensure and annual health assessment requirements
    D. Communication/Relationships
  21. Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others
  22. Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community
  23. Adheres to Continuum’s legal and ethical standards
  24. May be required to perform other duties as assigned
EDUCATION REQUIRED
High School Diploma
EXPERIENCES AND/OR SKILLS REQUIRED
Two years community health experience; Spanish speaking.
LICENSES/CERTIFICATIONS REQUIRED
