Job Description

II.   PURPOSE

  Is cognizant of the philosophy, standards, objectives and policies of the Department and the Hospital Center.

A. Clinical/Technical/Service

 

 

 

·         Demonstrates the ability to perform clinical/technical/service/administrative tasks:

 

 

 

·         Provides all field-based health education and skills building services to clients.Add additional Clinical/Technical/Service/Administrative tasks here.

 

 

 

·         Accompanies clients to primary care appointments and to other healthcare and social service encounters, as needed.

 

 

 

·         Coordinates ongoing navigation and logistical support for appointment keeping reminders, transportation and childcare arrangements.

 

 

 

·         Tracks the client’s progress through the health education curriculum.

 

 

 

·         Conducts social services and benefits reassessment and follow-ups.

 

 

 

·         Provides critical feedback to other staff members of the healthcare team based on his/her observations in the field.

 

 

 

·         Educates, coaches and empowers clients.

 

 

 

·         Observes and records client self-administration of ARTs.

 

 

 

·         Assess for and reports any ART related side-effects.

 

 

 

·         Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

IV.   EDUCATION REQUIRED

High School Diploma required; Bachelor’s degree preferred. 

V.    EXPERIENCES AND/OR SKILLS REQUIRED

Minimum of two years case management or related experience working with HIV/AIDS individuals, homeless or other underserved population.  Minimum of two years field work experience (includes home visits, escorting, outreach, etc.).  Excellent oral and written communication skills.  Excellent interpersonal skills.  Excellent computer skills including but not limited to knowledge of database systems and the ability to enter progress notes.  Ability to work within a team environment.  Ability to be flexible when asked to adjust schedule due to program needs.  Ability to assess needs of clients’ and develop comprehensive care plan.  Knowledge of HIV/AIDS resources, benefits and entitlements in Metro NYC area.  Bilingual English, Spanish a plus.

VII.  JOB HAZARDS/PHYSICAL DEMANDS

      Ability to travel within the five boroughs to conduct home visits.
