Job Description

What You’ll Do

A visiting MTM pharmacy clinical manager will conduct home visits for patients who have been recently discharged from hospital and at high risk of readmissions. The following clinical pharmacy services will be provided:

1.    A comprehensive medication therapy review; identification, resolution, and prevention of medication use and safety problems

2.    Resolution of any medication access issues

3.    Patient education regarding proper administration, monitoring criteria, and possible adverse effects of each medication and follow-up phone calls

4.    Assessment and reassessment of patient’s health literacy and self-sufficiency

5.    Identification of any risk factors that may contribute to readmissions such as fall risks or discontinued/expired medications

6.    Administration of vaccines (influenza or pneumococcal vaccine) if indicated

7.    Communication with other healthcare providers to ensure continuity of care regarding medications

8.    Serve as a member of the Mobile Acute Care Team (MACT)

 

 

Duties and Responsibilities

       Deliver MTM services, including but not limited to: comprehensive medication reviews, disease specific medication reviews, and targeted interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy and to improve clinical outcomes

       Assist with managing medication costs through cost-effective interventions which may include, but are not limited to, formulary or therapeutic interchanges, pill splitting opportunities, combination products and assessing unnecessary and/or duplicate medication therapies

       Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams

       Follow-up with patients and/or caregivers with an additional follow-up phone call subsequent to the initial in-person visit and appropriately document outcomes in a timely manner

       Serve as an advocate for the home care MTM pharmacy model to introduce and explain the benefits of MTM services to patients, family members and providers within the community

       Ensure patients and family members have a positive experience by providing a high level of customer service by always responding promptly, professionally, and politely

       Serve as an educational site preceptor for post-doctorate pharmacy residents and doctor of pharmacy students

       Assist in research programs relating to ambulatory care

       Provide education for medical, nursing, and pharmacy staff regarding medications and services

What You’ll Bring

·         Pharm.D.

o    New York State Licensure as Pharmacist with active registration

·         Completion of PGY-1 and PGY-2

·         Knowledge and understanding of the Core Elements of MTM services:

1)      Medication Therapy Review Comprehensive and Targeted

2)      Personal Medication List (PML)

3)      Medication-Related Action Plan (MAP)

4)      Interventions and Referrals

5)      Documentation of Services and Follow-up

·         Excellent listening skills present a warm, friendly, and empathetic demeanor demonstrating active listening techniques

·         Excellent interpersonal skills demonstrate empathy with patients and family members in tone, language, questions and demonstrate patience, caring, sensitivity, empathy, and cultural competency throughout job responsibilities

·         Conflict resolution skills able to defuse potential customer or patient conflict utilizing the appropriate tone, support, questions or counsel and respond in a supportive manner
