Job Description


A. Clinical/Technical/Service

  1. Serves as clinical and administrative nursing and interdisciplinary leader for the practice. 
  2. Responsible for assessing, identifying, planning, implementing and evaluating the processes, technology, personnel, and facility needs required to achieve patient outcomes safely and cost effectively in a patient centered environment

3.     Acts as a member of the patient care service Leadership Team and collaborates with and guides the interdisciplinary team members in establishing and achieving patient outcomes

4.    Speaks to the vision and mission of the department and organization; inspires commitment essential to achieving the organizational and departments mission, goals, standards, policies in a patient centered model of care delivery system

5.     Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, spiritual, functional, financial and educational, safety, and related criteria, appropriate to the age of the patients served in assigned area

Age Specific Competencies (Neonate/Infant, Pediatric, Adolescent, Adult, Geriatric)

a.   Identifies physical, behavioral and emotional characteristics typical for the age group

b.  Modifies approaches based on patient age-specific needs and responses to treatment

c.   Provides care for patients based on age-specific needs

d.  Uses communication techniques which are age appropriate

B. Clinical Practice


Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement.


Assesses the health status of patients/families by means of comprehensive health history and physical examination.


Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area.


Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.


Documents findings and plan of care in the patient's medical record.


Consults with physician as established in the collaborative practice agreement.


Initiates referrals to specialty services and fosters continuity of care.


Participates in team meetings and conferences to enhance an interdisciplinary approach to acute/primary health care delivery.


Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months


Manage a clinical specialty program for a select patient population across the hospital continuum and provide outcome data for program evaluation and performance improvement.


B. Organizational/Managerial

6.     Maintains patient/employee confidentiality in the management of information

7.     Assesses data to determine the process, technology, personnel and facility needed to deliver care in a designated patient care area (MSHS Radiation Oncology Role)

8.     Formulates objective, measurable goals consistent with the Hospitals’ mission, vision and patient care principles

9.     Interviews, hires/terminates, disciplines, rewards and evaluates patient care staff

10.  Conducts care team performance evaluations and facilitates peer evaluation

11.  Analyzes quality and operational metrics compared to established benchmarks; leads interdisciplinary team in developing and refining strategies to achieve measurable outcomes .


13.  Modifies delivery system as indicated

14.  Monitors selection and use of unit supplies, technology, equipment

15.  Develops and manages the unit budget as established

16.  Promotes fiscal awareness and accountability of all staff

17.  Utilizes appropriate resources and departments to facilitate unit operations

18.  Participates in a unit based QI program

19.  Achieves successful compliance with regulatory agency standards

20.  Assures staff understanding of and compliance with infection control mandates

21.  Conducts/facilitates leadership and care team meetings, demonstrates ability to achieve meeting outcomes

22.  Maintains records of minutes

23.  Assures that the ability of assigned patient care provider is congruent with patient/family health care needs

24.  Identifies and follows through with consequence management

25.  Facilitates and promotes staff participation in research activities appropriate to specialty area as available

C. Educational/Professional Development

26.  Participates in the development and revision of policies and procedures

27.  Meets regulatory, licensure and annual health assessment requirements

28.  Identifies learning strengths and needs of staff /self and utilizes learning resources

29.  Provides presentations and educational offerings

30.  Participates in special projects and committees

31.  Attends relevant workshops, conferences and seminars

32.  Obtains feedback from leadership team and care team on her/his effectiveness

33.  Assures staff development by providing learning opportunities within and outside the organization

34.  Participates in the planning and evaluation of educational programs to meet unit needs and to promote staff development

35.  Develops, implements, and evaluates the unit-based orientation program for new employees

D. Communication/Relationships

36.  Demonstrates a professional, courteous, and respectful attitude in dealing with patients and families

37.  Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community

38.  Practices as a member of the interdisciplinary care team via effective and respectful communication and collaboration

39.  Assures that all unit staff are informed of medical center’s developments and participates with the patient care area planning and problem solving

40.  Promotes motivation and productivity through the application of sound employee relations activities, labor relation principles and clear communication

41.  Recognizes and rewards care team achievements

42.  Communicates care area purpose and goals

43.  Adheres to Continuum’s legal and ethical standards

44.  May be required to perform other duties as assigned

NYS RN and FNP or PNP with Master's degree, DEA, BCLS and At least 2 years of exp in pediatrics.  NP exp preferred.
