Job Description

IDENTIFYING INFORMATION

Position Title:  Patient Navigator

Department: Cardiology 

Reports to:                                             

RESPONSIBILITIES

A. Clinical/Technical/Service

 

 

 

1.     Demonstrates the ability to perform clinical/technical/service/administrative tasks:

 

 

 

2.     Communicates with residents and physicians to identify patients who require surgery

 

 

 

3.     Communicates with patients, verifying insurance and other demographic information

 

 

 

4.     Liaison to physician office/practice in regard to information on ER and other inpatient admissions to include providing feedback, notes, misc information as appropriate

 

 

 

5.     Confirm and/or verify that proper referrals/authorizations are in place for visits/admissions/etc

 

 

 

6.     Make all necessary appointments required so the patient can be medically cleared for surgery

 

 

 

7.     Monitors patients appointment and follows-up with patient if they do not show for appointment

 

 

 

8.     Facilitates information to referring physician regarding status of patients scheduled for surgeries or procedures

 

 

 

9.     Maintains a database that on captures all relevant steps necessary in process for surgery

 

 

 

10.  Database is stored on shared drive so medical staff can view process and patient progress

 

 

 

11.  Database is updated as needed

 

 

 

12.  Is actively involved in physician outreach to promote and educate physician offices as to services offered.

 

 

 

13.  Will work as a liaison to the physicians, patients, and clinical services department internally and externally

 

 

 

14.  May perform other functions as requested by manager

 

 

 

15.  Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

20.  Meets regulatory, licensure and annual health assessment requirements.

 

 

 

21.  Identifies learning strengths and needs

 

 

 

22.  Utilizes learning resources.

 

 

 

 

D. Communication/Relationships

 

 

 

23.  Demonstrates a professional, courteous, and respectful attitude in dealing with patients,

      families and significant others.

 

 

 

24.  Displays courtesy, tact and patience during interactions with all members of the hospital staff

      and extended community.

EDUCATION REQUIRED

      College preferred; 3 years of office/phone and customer experience; medical background preferred.

      Good and clear communication and writing skills. Good interpersonal skills and ability to multi-task. Bilingual (English/Spanish) required.
