What You’ll Do:
  • Works closely with Care Transition Team to obtain insurance authorization for patients prior to discharge
  • Verifies/Interacts with Insurance: clarify covered benefits, contact providers and obtain authorization numbers for insurance companies for post-acute services.
  • Assists with facilitating successful discharge plans which are critical to LOS reduction and patient throughput
  • Assists Case Management and Social Work in locating resource information and responds to inquiries about a broad range of community services, eligibility requirements and application procedures.
  • Develops professional relationships with all levels of staff and interdisciplinary team
  • Communicates effectively with Care Transition Team to ensure a timely discharge
  • Prepares all discharge planning packets for appropriate post-acute services
  • Sends clinical information to insurance companies through ACM for discharge planning and authorization
  • Maintains contact with the unit case manager in respect to timely provision of documentation
  • Maintains contact with the unit social worker in respect to patient choice and accepting facility
  • Uploads updated clinical information from PRISM into ACM
  • Follows escalation protocol for difficult cases needing assistance from leadership team
  • Updates insurance companies contact list to ensure information is up to date
  • Schedule transportation for patients being discharged home or to skilled nursing
  • Uploads updated clinical information from PRISM into ACM
  • Uploads denial letters into ACM and send them to accepting facilities
  • Maintains Weekend UM/CM task list and provides progress notes daily to the rest of the team
  • Keeps a log of difficult cases making note of number of days authorization was delayed and reason why
  • Communicates issues affecting the department’s operation to the Administrative Manager
  • Utilizes e-Paces to find out proper patient insurance information
  • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area
What You’ll Bring:
  • High School Graduate or equivalent. Bachelors preferred.
  • Minimum one year secretarial experience. 
  • Medical terminology preferred.  Ability to use PC software (Word for Windows) preferred. 
  • Spanish speaking preferred.
  • Discharge planning, case management, utilization review experience a plus.
