What You’ll Do:  

The Mount Sinai Hospital Delirium Program provides assistance to our medical and surgical care teams by facilitating the early identification and treatment of Delirium in our hospitalized patients. The Advanced Practice Nurse Practitioner with the Hospital Delirium Program is a Nurse Practitioner with advanced clinical knowledge and skills who is responsible for working with interdisciplinary team and collaborates with the Director and the Associate Director of Hospital Delirium Program in the implementation of the hospital wide Clinical Delirium Program. The Advanced Practice Nurse Practitioner upholds patient care standards, and ensures quality in the care of hospitalized patients and their families. The Advanced Practice Nurse Practitioner supports the interdisciplinary Delirium Team in the prevention, assessment, management and treatment of Delirium in patients across the hospital and will oversee staff.
  • Assesses the health status of patients/families by means of comprehensive health history and physical examination.
  • Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area.
  • Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.
  • Documents findings and plan of care in the patient's medical record.
  • Consults with physician as established in the collaborative practice agreement.
  • Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months
  • Advocates for patients/families and supports patients' rights.


What You’ll Bring:
  • NYS RN and NP with Master's Degree, DEA , BCLS and ACLS. New grad NP with at least 2 years exp in acute care nursing will be considered. NP exp preferred.
