Roles & Responsibilities:

A. Clinical/Technical/Service

1.     Assesses the nutritional status of patients and manage the nutritional care process, including evaluation and monitoring

2.     Educates patients, physicians and other health professionals

3.     Screens, assess and documents nutritional status of patients; assigns appropriate level of care

4.     Responsible for planning, development, implementation and evaluation of the process of nutrition care plans

5.     Plans, implements and evaluates nutrition education for patient and significant other(s)

6.     Develops and revises nutrition education tools

7.     Documents nutrition care provided in patients’ medical record in accordance with departmental standards

8.     Assists in nutritional analysis of menus and modified diets and revision of diet manual

9.     Conducts meal time visits and food intake analysis as deemed necessary

10.  Participates in interdisciplinary patient care rounds and activities

11.  Plans, Develops and participates in special projects and committees

12.  Acts as consultant to physicians, nurses, social workers, medical students, and other disciplines. Presents classes and lectures on nutrition related topics to health care team

13.  After patient interviews, records pertinent eating habits on Kardex file and give to menu the supervisor to use in preparing patient menu if necessary.

14.  When menu supervisor not on duty, prepares any necessary menu/s and submits to Diet Clerks in Central Diet Office

15.  Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area

a. Identifies physical, behavioral and emotional characteristics typical for the age group

b.Modifies approaches based on patient age-specific needs and responses to treatment

c. Provides care for patients based on age-specific needs

d.Uses communication techniques which are age appropriate

B. Organizational/Managerial

16.  Participates in the department’s performance improvement activities

17.  Maintains patient/employee confidentiality in the management of information

18.  Maintains effective communication and close relationship with departmental administration and managers to identify and solve problems

19.  Communicates appropriate information to patient services and/or management personnel

C. Educational/Professional Development

20.  Participates in the development of other staff members

21.  Meets regulatory, licensure and annual health assessment requirements

22.  Identifies learning strengths and needs

23.  Utilizes learning resources.

24.  Checks schedule of lectures given within the Hospital Center; attends those with nutritional impact

25.  Keeps abreast of new trends and developments in the Nutrition field and utilizes knowledge in current practice

D. Communication/Relationships

26.  Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others

27.  Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community

28.  Adheres to Continuum’s legal and ethical standards

29.  May be required to perform other duties as assigned

Requirements:

EDUCATION REQUIRED

B.S. Degree in Food and Nutrition. Completion of approved American Dietetic Association program for qualifying experience, AP-4 Program or Internship Program.

 

EXPERIENCES AND/OR SKILLS REQUIRED

2 Years Clinical Teaching- 3 Years Hospital Clinical Experience.  Maintain Registered Dietitian status through Continuing Education Programs.

 

LICENSES/CERTIFICATIONS REQUIRED

Member of American Dietetic Association. Registered Dietitian (RD).
