Job Title: Pharmacist III PTR



Roles & Responsibilities:

·         Demonstrates the ability to perform clinical/technical/service/administrative tasks:

·         Ensures rational drug therapy by reviewing inpatient and outpatient medication orders prior to dispensing.

·         Evaluates new orders against existing patient profiles and provides appropriate pharmaceutical care.

·         Demonstrates the ability to detect, communicate and avoid drug interactions and the administration of drugs to which the patient may be allergic.

·         Contacts prescribers for issue resolution.

·         Dispenses prescription medication to inpatients, outpatients and employees as per departmental policies.

·         Appropriately supervises non-licensed personnel.

·         Appropriately checks all work conducted by non-licensed personnel requiring direct pharmacist supervision and authorization, e.g., unit-dose cassette filling, I.V. batching.

·         Dispenses and maintains accurate records of dispensing controlled substances, tax-free alcohol and investigational new drugs.

·         Demonstrates ability to operate required equipment, e.g., unit-dose repackaging, total parenteral nutrition compounder, laminar flow hoods, etc.

·         Compounds products as required, including sterile parenteral products, maintaining required logs.

·         Initiates and documents pharmacist interventions to change prescribing where required.

·         Investigates and documents reports of Adverse Drug Reactions.

·         Ensures an adequate inventory of medications and supplies is maintained. Arranges for emergency inter-hospital transfer of medications when required.

·         Completes all required documentation.

·         Maintains a safe, clean working environment.

·         Maintains and enforces appropriate security measures at all times.

·         Prepares required reports of pharmaceutical services.

·         Provides drug information services to other health care professionals. Demonstrates ability to retrieve and convey information from various sources, e.g, Trissel, AHFS, Micromedex, etc.


·         Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

B. Organizational/Managerial

·         Participates in the department’s performance improvement activities.

·         Acts as the Director's agent in the absence of on-site managers, e.g., evenings, nights and weekends.

·         Exercises sound judgement, recognizes and attends to priorities.

·         Participates in departmental quality assurance activities.

·         Responds appropriately to supervision provided by Pharmacy Directors and Managers.

·         Demonstrates understanding of and compliance with all departmental, hospital, state and federal policies and regulations.

·         Offers recommendations for improving efficiency.

D. Communication/Relationships

·         Demonstrates a professional, courteous, and respectful attitude in dealing with patients,

      families and significant others.

·         Displays courtesy, tact and patience during interactions with all members of the hospital staff

      and extended community.

·         Advises management staff immediately of any discrepancies in controlled substances or other inter-departmental issues requiring immediate attention.

·         Advises management staff immediately of any pharmacy dispensing errors. May assist in contacting patients only with the approval of a departmental director and the Director of Risk Management. Ensures prescriber is alerted if any patient has received an unintended product, dose, duration or route of administration.

·         May be required to perform other duties as assigned or perform other duties when necessary.




·         Bachelor of Science Degree from an accredited college of pharmacy.

·         Pharmacist - previous hospital experience preferred but not required.

·         Senior Pharmacist I - two years hospital experience required.

·         Senior Pharmacist II - four years hospital experience required.

·         English proficiency, oral and written, required for all categories.
