Job Title: Social Worker - Medical / Surgical  

 
Roles & Responsibilities:

Discharge planning for adult patients on the medical and surgical floors.


Perform psychosocial assessment of high risk patients.


Provide individual and family education and counseling.


Set up, coordinate, participate or lead patient and family meetings if needed.


Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.


Evenings: 4-10pm 


***occasional weekends required***


 

Requirements:

EDUCATION REQUIRED
Masters in Social Work from an accredited school of social work.
LICENSES/CERTIFICATIONS REQUIRED
NYS Certified Social Worker
