Reviews and classifies clinical data from the patients records for the Hospital Center’s reimbursement administrative performance improvement systems
Reads medical records including discharge summary, progress notes, consultations, operative reports, diagnostic laboratory reports etc. to ensure appropriate diagnoses and procedures are documented in inpatient, ambulatory surgery and emergency department patient records
Applies uniform discharge data set (UHDDS) definitions to select the principal diagnosis and sequence secondary diagnoses, complications, co-morbidities and procedures as documented in the medical record
Utilizes available tools, both manual and electronic,to code diagnoses and procedures in accordance with the established international classification of diseases (ICD-9-CM) and Common Procedural terminology (CPT-4), principles and conventions
Abstracts specified elements for the completion of the medical record abstract
Enters data into a CRT terminal for inclusion in the hospital center’s data base, assignment of diagnostic related groups (DRG) classification, transmission to the patient’s bill, submission to NYCHSRO,Sparcs case mix, indices and NYSDOH registries
Prints attestation as needed in accordance with departmental policies and procedures
Codes and abstracts inpatient, ambulatory surgery and emergency room records
- High School diploma or equivalent required with a comprehensive knowledge of medical terminology, anatomy and physiology, ICD-9-CM and CPT-4 coding
