Medical Director Next-Generation Intensive Outpatient Care Program

This new primary care practice will do just that. The parties want to change the pattern, so it’s not about treating patients in a hospital but rather keeping them healthy and happy at home – the right care, at the right time, in the right place, every time – delivered by an interdisciplinary team of professionals. We seek a medical director to lead this key transformation initiative and ultimately pave the way for a new model of patient care focused on population health management.
The team-based nature of this practice allows the medical director to train and lead the entire team, including the physicians, nurse practitioners, social workers, care coordinators, licensed practical nurses, health coaches, and administrative staff who work with patients and their caregivers to increase engagement and motivation.
You will ensure ready access to excellent patient care through top-notch customer service, better health outcomes, and lower health care costs.
Who We Are Looking For
The Medical Director needs to be a driven physician with a clear vision for how to advance clinical care that works towards the Triple Aim. This position is ideal for a results-oriented individual who embraces innovation and enjoys practicing in a changing and highly collaborative environment.
Requisite qualifications include board certification/eligibility in internal medicine, medicine/pediatrics or family medicine. Proficiency in Spanish a plus.
